USDSA Law Enforcement Recognition Program Nomination Form.
The forms can be found in The Deputy Sheriff Magazine, upon request from the USDSA
or through the link provided below. Self-nomination is permitted.
As with all of the USDSA’s programs, there is no charge to nominate someone for an award,
or to receive the award.
